Privacy Policy

At Blooming Well, your privacy is our top priority. We are committed to providing a safe, respectful, and confidential space where you can feel secure sharing your thoughts, emotions, and experiences. This Privacy Policy outlines how your personal information is collected, used, and protected, in compliance with the standards set by the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and applicable privacy legislation, including the Personal Health Information Protection Act (PHIPA).

Confidentiality

All information shared during sessions is strictly confidential and will not be disclosed without your written consent, except in the following legally required situations:

  • If there is a risk of serious harm to yourself or others.

  • If there is suspected or disclosed abuse or neglect involving a child or vulnerable person.

  • If your records are subpoenaed by a court of law.

  • If there is a legal obligation to report misconduct by a regulated professional.

Outside of these exceptions, your privacy is fully respected and protected at all times.

Collection and Use of Personal Information

We collect only the information necessary to provide safe, effective, and personalized services. This may include:

  • Your contact and identification details

  • Relevant health, emotional, and psychological history

  • Session notes and treatment progress

This information is securely stored and is only accessible to the practitioner. We do not share, sell, or disclose your information to third parties without your consent, unless legally required.

Your Rights

As a client, you have the right to:

  • Access your records upon written request

  • Request corrections to inaccurate information

  • Withdraw consent for the use or disclosure of your information, unless required by law

Professional Standards

As a registered Social Service Worker, I strictly follow the Code of Ethics and Standards of Practice set by the Ontario College of Social Workers and Social Service Workers. This includes:

  • Maintaining confidentiality and professional boundaries

  • Practicing informed consent

  • Promoting dignity, respect, and autonomy for all clients

Record Retention

Client records are securely stored and retained in accordance with legal and regulatory requirements. Records are kept for a minimum of 10 years from the date of your last session, or 10 years after a minor reaches the age of majority, whichever is longer.
